medical examination report to include in AOS?

Hello guys

i did not really understand, do i need to include my medical examination report while applying for AOS?

i do not have any medical examination report cause the embassy did not return it back to me neither the border officer did

please any advise will be appreciated

You are only required to submit a vaccination supplement form DS-3025 with your AOS. IF the DS-3025 is completed properly, this is all you need to submit. If it is not completed properly you will need to visit a civil surgeon and have your vaccines transcribed onto an I-693.

A properly filled out and completed DS-3025 should have the following:

• All vaccines are marked as either complete, not age appropriate, insufficient time interval
• It is marked at the bottom as either Complete, or Incomplete but may be eligible for a blanket waiver.
• Signed by the doctor

i dont think i need to submit the medical examination record, this is what i found:

I-693, Medical Examination of Aliens Seeking Adjustment of Status.I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy).

and USCIS web site says:

• Form I-693, Report of Medical Examination and Vaccination Record, if applicable
