So, he has no copy of the DS-3025 vaccination worksheet? If you are absolutely sure he had all the required vaccinations and they were marked off on the DS-3025 at the time of his medical, then you can add a statement that all his vaccinations were complete at the K-1 medical and the documents should be included in his file that he turned in at his POE. It is no guarantee that he will not get an RFE, but if it goes through like that, then you won't need to see a civil surgeon. Worry about that if you happen to get an RFE for it.

Thank u for answering my question, actually i am not really sure that all the required vaccinations are marked off, the civil surgeon doctor in his country who completed the DS325 form,he didn't tell my husband anything,he just checked him up and made him an X-ray,the thing he told him that you are healthy and nothing to worry about, then he asked him to come next day to get the sealed envelope, therefore my husband has no clue what is complete or not complete inside the envelope,i don't know what to do,i am very confused!!!

You should go read up this guide.

http://www.visajourney.com/forums/topic/235331-aos-ead-ap-from-k1-the-aussie-way-doc-list-included/#entry3607999

You don't have to include your passport photos but you need to file I-864 - Affidavit of Support (completed by US spouse) in AOS.

Read up the instructions for I-864 and the documents to be included.

The latest fee for AOS is I-485 $985 + $85 biometric fee for a total of $1070
