Gearing up to send off my AOS Package - Have a checklist with a few questions

Just wondering a few things. For starters, I remember, when my wife submitted my initial K-1 petition, that we weren't supposed to bind the package using staples, but instead had to use one of those big black paperclips. Is that still the case?

Also, on top of sending my vaccination records, my immigration physical doctor sent me along with a CD of my chest x-rays. Am I supposed to send that as well as my DS-3025? Also, I see on the Visajourney Checklist that I am supposed to have an I-693? Is that true? I thought I was just supposed to have my DS-3025, and that my immigration physical would have been sent off with my package when I crossed the border?

If I have to send the Chest X-Rays, how should I package them? Should I somehow affix the CD in it's paper holder onto a larger piece of paper?

Also, could someone confirm that I have what I need? I have the following, ready to send away:

Passport, W/ Visa and stamps copied (I just need my biographic page, my Visa page, and the pages w/ stamps relevant to my recent K-1 entry, correct? I also have a copy of my paper I-94 stapled into my passport).

Electronic I-94 Copy
NOA2 copy
Marriage License - certified copy
Vaccination Report (DS 3025) and Chest X-Rays on CD
Cover Page (Should this just look like the K-1 cover page? Does anyone have an example of this?)

Passport Photos: (Should these be included in my package the same way that they were included in the K-1 petition package?)

I-864 x 2, one from my Wife and one from my Mother in Law:
Letter from Wife's Employer, and two recent paystubs. Letter explaining why no tax return filed
- Letter from Mother in Law's employer, recent paystubs, last 3 years of tax returns. Birth Certificate from Mother in Law

I-131
G-1145

Payment in cheque form. ($1070, correct)

Is there anything I am missing here? Any help with any of my questions would really be appreciated. Thanks a ton!

Yes, same tips for mailing thing to the USCIS > http://www.uscis.gov/forms/forms-and-fees/general-tips-assembling-applications-mailing

No x-ray CD needed. That is yours to keep. The USCIS does not accept any forms of digital media anyway. See this post for whether or not you need an I-693 > http://www.visajourney.com/forums/topic/428381-i-693-report-of-medical-examination-and-vaccination-report/?p=6206232

You need the supporting documentation for the I-765 and I-131 too, including the 2 passport photos for each of those. That is a total of 6 passport photos needed. And yes, cover letter is basically the same as the K-1 cover letter. You just list what documents are included in the order you placed them for mailing.

Thanks for the help, as usual Jay-Kay. I appreciate it a lot. I have a few more questions...

So for my I-131, I need my Biographic page of my passport, my Visa page, and my entry stamp? For the I-765 I need only my passport biographic page?

Does that mean I'll need three copies of my passport's biographic page, and 2 copies of my Visa page and entry stamps? Or will 1 copy of each do? I'm confused in areas where there is overlap in terms of required evidence.

Other than that, is there anything else I need to grab, or does it all look good?

Yes, you need the bio page/K-1 visa/entry stamp copied 3 times, and 6 passport photos. Just place what each form need behind that particular form to keep everything organized, then list the order you placed them in on the cover letter.

Besides those extra copies, you seem to have included everything else that is required.
