I have finished putting together my AOS paperwork but the medical record side of things is still a bit hazy to me.

I had my visa medical done in mid September 2013, everything went fine and I didn't need any updates or additional shots, the one I did need I got done for free at my doctors before I went to the medical.

My medical is obviously well within a year of travelling/marrying/filing for AOS and all that, so from what I have read around the forum and guides, is there really nothing more I need to send in the AOS paperwork regarding this? That USCIS will have my medical information already from the envelope I gave them on entry to the US? I have copies of the documents and receipt from my medical in London and the vaccination record from the doctors clinic, but I wouldn't want to send it if it really isn't necessary.

Make a photocopy of that DS-3025 vaccination form Knightsbridge gave you. Send it with AOS. Your copy is only a photocopy of the original which is with your other medical records and will (hopefully) be added to your AOS case file. Sometimes that medical record does not get matched up. Nothing you can do to prevent that really.

If you want to add an extra layer with that DS-3025 photocopy, you can attach a short statement with it saying you had your visa medical exam on [date] and are including a copy of the immunization record DS-3025 showing you were complete for all vaccinations required for adjustment of status on that date. Just a little extra icing on the cake to connect the dots for them.

Thanks for the tip. Yeah I am sending them a copy of that form as well as a copy of the vaccinations list my doctor in the UK printed out for me. So there's no need to send them/fill out anything on an I-693 form at all? Just want to double check before I mail all this stuff off.

No, don't send your NHS records. They only consider something signed by a US government approved Panel Physician (Knightsbridge) or a Civil Surgeon. You have all shots documented by a Panel Physician and they should accept that. They did ours and many, many, many others here on VJ. Not ever a 100% that the person who picks up your file knows their rules, but it is definitely worth a shot. No pun intended.
