Hi,

So I'm preparing my documentation for the Adjustment of status and came across the medical/above form, and I just have a couple questions about the vaccinations.

I've read that the DS-2053 has to be 'perfect', otherwise I have to go to a civil surgeon with the above form. At my medical for the K1 visa all but three vaccines were ticked as 'not age appropriate'. The other three were MMR, tetanus and varicella. I received the MMR and tetanus vaccines from my GP before I left for the US and have proof of this (signed and stamped Immunisation report).

1. Do I need to get the varicella vaccination if my DS-2053 says "VH"?

2.Do I need to go to a Civil Surgeon to get the above form signed off, or can I just submit my DS-2053 with my immunisation report?

VH means Varicella History and that is sufficient for the DS-3025. I'm not 100% on question 2, but if you mean you got your vaccines AFTER your medical but before POE and they're not marked on your DS-3025, it would most likely need transcribing to an I-693 by a civil surgeon.

I agree with QueenofBlades. If the vaccines were done outside of Knightsbridge and are not reflected on your original medical paperwork, you should take the vaccine record from the medical as well as your GP to a civil surgeon. You'll have to pay them to transcribe the info onto a I-693 so that the form is complete.

Call around before you go in and inquire about who has the lowest cost to just transcribe the forms. You shouldn't need a whole new medical (no new x-ray and such), so be sure to tell them what you need and ask for a price. You'd be surprised at how drastically different policies and prices will be across civil surgeons.

You said you had all but 3 vaccines marked as 'not age appropriate', correct? Then at the bottom of your DS-3025 form is the following marked?

  • Vaccine history incomplete...Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate

If it says you are eligible for a blanket waiver, you do not need to send in form I-693 with your AOS application. Just send in a copy of your DS-3025 form.

1. No, you had chickenpox. Shot waived

2. Yes to Civil Surgeon because USCIS doesn't take signed proof except from a Panel Physician (Knightsbridge) or a civil Surgeon. Your DS-3025 is incomplete.

This is not correct. The wording is MAY be eligible for a blanket waiver.it doesn't say absolutely eligible for blanket waiver on everything. Yes the ones marked age not appropripriate will be waived, but there are two shots not listed (MMR and Tdap) on her DS-3025 so it is incomplete and won't work for AOS.

Thanks, I figured that would be the case. Will I have to get those two shots again, or will the civil surgeon just sign me off? Also, do I need to send off any proof of the previous medical?

You should have a copy of your vaccines from Knightsbridge. Take that with your record of the additional vaccines from your GP. The new I-693 will be complete and replace the original (the doctor at Knightsbridge filled out a I-693). Basically, the I-693 you get filled out in the states will have everything and more than the original you did in the UK. It will be filled out by the civil surgeon and placed in a sealed envelope. The sealed envelope with the I-693 will be sent with your AOS materials.

You should not get more shots. It's not necessary and probably not healthy! As long as you have records of receiving them, you'll be set when you go to the civil surgeon.

The attitude of civil surgeons vary greatly. The two I talked with would do nothing for me unless I agreed to the full exam plus immunizations to the tune of $700. Others have posted being able to get just the shots or just the transcription.

Call around your local community and surrounding communities.

Ahhh sorry I misunderstood. I thought the mmr, td and vh were actually the only 3 that were listed with all others as not age appropriate. So by this yes the ds-3025 is incomplete.

Nothing from your K1 medical at Knightsbridge. Just the new, 100% complete! sealed new I-693!

Just to say it over again, a civil surgeon can look at any number of records---from your childhood list, the Knightsbridge DS-3025 sheet, the new shots, a US doctor, a health department or a drugstore clinic. He will combine everything onto an I-693. It is now flu season, so you will also need a flu shot. Call around and find the cheapest place and take the proof along with the other proofs.

Read the instructions for the I-693 where it says you do not need another medical exam. You also don't need a TB skin teat. You were cleared of TB with your chest x-ray and that is written in your medical report turned over POE. That medical record gets matched up with you AOS file. So there s nothing to send from Knightsbridge. When you start calling, if they don't act like they understand the CS only needs to fill out the immunization part of the I-693, then hang up and call the next one.
