my wife doesn't have a record of her vaccinations from abroad and she was only given MMR and Varisella during K1 abroad.

Does she need to have all vaccinations done in the form ?

I went to a civil doctor by my home and he asked me a hefty fee of $350. How can I get this done for cheaper?

Also, I asked my primary care physician to do it ( I am not sure if they verify that the person is a civil surgeon in their list mentioned), but he doenst' know if she really needs all vaccinations. He is willing to give all vaccinations but I am not sure which ones we need.

Any help would be appreciated.

If your wife received a copy of the "Vaccination Worksheet" at her K-1 Visa medical exam, you can have the information transferred onto the Vaccination Supplement by a civil surgeon. We were charged $35.00 for this service.

If your wife's K-1 medical exam was less than one year ago, she does not require another medical exam. She only requires the Vaccination Supplement.

The Vaccination Supplement has to be completed by a Civil Surgeon (approved by USCIS), not your own family doctor.

Whatever your wife used to prove what vaccinations she had in her home country, she could use at the Civil Surgeons if she doesn't have a copy of the Vaccination Worksheet from her K-1. Or, maybe she could try to email, phone or write the doctor where she had her K-1 Visa medical exam to see if they could forward you a copy of her vaccination worksheet.
