Attention All British K3 People

I am trying to find out of this checklist is the one that the U.S. Consulate of London mails out to you as part of the packet 3:

...and as a follow-up, what exact forms need to be returned to the Consulate when getting packet 3. I know that the DS-156 x 2 copies, the DS -230, but that checklist, I have no clue. The website has no details of the packet 3 in relation to the K3 visa, though does have words about the K1:

It talks of the IV-15k, which, of course, is not available on their website. Anyone can shed light on this? Thanks!
