I think it depends on who processes your case. Some get no RFE and others do get one if even the slightest thing is not correct on the DS-3025. You can always send it in as is and then if you get an RFE, worry about getting the I-693. If you don't get an RFE, then great! No wasted time and money. Others feel it is best to get the I-693 to begin with and play it entirely on the safe side.

Rotovirus is commonly left unchecked. We sent the 3025 and the instructions and we were not even asked about vaccines. Joan's GC is scheduled to be delivered today. The key word in the instructions is if the form is filled out "correctly" not "completely." Is the "blanket waiver" box checked?
