Hello,

Have you filed for AOS? did you have to seee a civil surgeon before you send your AOS forms sent?

We didn't. We just submitted her physical/vaccine paperwork from her K-1 process. We passed through AOS with no issue. HOWEVER, the guides specifically tell you not to do this. And I'm sure there was less of a problem considering her physical and vaccinations were done in Canada by a Canadian doctor. Maybe not. We read a lot and despite the guides telling us we needed another civil surgeon to sign a certificate for us, we decided not to and risk RFE.

Really, an INTENT TO DENY ? That is crazy. Like said it should have been an RFE.

Reading on vj you will find a fair number that when they tried using the DS-3025 they got an RFE requiring the I-693.

We just sent the DS-3025 but included the specific instructions from the USCIS webpage. No issue at all. It wasn't even mentioned. Joan's Green Card arrived today. I suppose that it ultimately depends on who reviews your file but we were not going to pay over $200.00 for a CS in Hawaii unless we had to (yup, $200.00+ just to transcribe the information...others insisted on unnecessary vaccines and full medicals). It worked out for us and several friends who have done the same thing. It is a decision that only you can make for yourselves.

This post is quite helpful in explaining what a properly filled out DS-3025 should look like. It is not all about having the blanket waiver checked off. > http://www.visajourney.com/forums/topic/428381-i-693-report-of-medical-examination-and-vaccination-report/?p=6206232

Yes but it also had all of the information marked as noted in the link provided by Jay-Kay...

EXCEPT rotavirus was not checked off:

"2. Everything else marked not age appropriate, insufficient time interval, not flu season...things that allow a waiver."

Hence the quandry.

I think it depends on who processes your case. Some get no RFE and others do get one if even the slightest thing is not correct on the DS-3025. You can always send it in as is and then if you get an RFE, worry about getting the I-693. If you don't get an RFE, then great! No wasted time and money. Others feel it is best to get the I-693 to begin with and play it entirely on the safe side.

Being that I found a Civil Surgeon in San Diego that will transcribe it for only $50, I'm going to play it safe and do that.

If you can find a CS that will do just the vaccination transcription for a good price, then I think playing it safe is probably the way to go. Some just find it really hard to find a CS that won't charge them less than $200 or so.

I just did a google search for rotovirus (I did not know what it was!) It is a virus that affects almost all infants and children (severe diarrhea) but not a big deal for adults (similar to a stomach flu)--each time you get this as a kid you build up your immunity to it (http://en.wikipedia.org/wiki/Rotavirus http://www.mayoclinic.com/health/rotavirus/DS00783). Vaccines are available for children but hand washing is the best prevention for adults. That's probably why it is not checked our sheets. The CS would have been a huge waste of money for us but we might have gone for it if we found one cheap enough. It seems to be an easy fix though if you did get an RFE for the CS.
