If the medical you took at your home country was not within a year of filing for the AOS or was incomplete, namely the vaccination form.

My fiance is not here yet, but we will be filing next month when she is. She took the medical in Manila in July, so she will be able to use that for her AOS and does not need to take another one. We wouldn't even see a civil surgeon if her vaccination form was properly filled out, but it looks like the doctor at her medical forgot to check one box that was supposed to be checked, so we will pay a Civil Surgeon $50 here in San Diego to transcribe her vaccination form to the i-693, to be sent in with the AOS.

Rotavirus, correct? Seems to be common. Some on here said that it caused RFEs, some got by with it. We are not taking our chances and will just pay the $50.

If you got the Flu shot before or during the medical in the Philippines you do not need to get another medical.

Well that is sort of a tricky question with the K-1. The instructions for the I-485 state you should be fine with sending a copy of the DS-3025 as long as you file within a year of your medical. BUT many of the adjudicators (are not well trained ) at USCIS look at the K-1 AOS application as they do other types of visas and expect to see the I-693 instead... this is why you may need a civil surgeon - to get the DS-3025 transcribed to the I-693.

You could take the gamble of simply sending in the DS-3025 (as that is the correct way of filing) and wait to see if you get an RFE for the I-693 or not. I have suggested to a few to include a copy of the page from the I-485 instructions with the requirements for the K-1 highlighted (to educate the adjudicator at USCIS) attached to your copy of the DS-3025.

The decision is up to you.

We are doing that Hank.. We are still finishing all this application papers, thourough checking every now and then, hopefully we can submit soon, of course double the works since I have also a k2. But we decided not go for civil surgeon as I have read that within one year thing and you'll be alright . But that rotavirus is bugging me and yes it's intentionally left blank. Hopefully no delays if ever.

Rotovirus is commonly left unchecked. We sent the 3025 and the instructions and we were not even asked about vaccines. Joan's GC is scheduled to be delivered today. The key word in the instructions is if the form is filled out "correctly" not "completely." Is the "blanket waiver" box checked?

We gambled with just sending in our fully complete DS-3205 and received an "intent to deny notice". It should really have been a RFE. Anyhow, it took us a week to respond to the notice and within four weeks of the notice we had our green card!
