They have changed the "status updated date" of my case yesterday on 06-June-2014.

i don't know is it good sign or bad. but atleast they did something.

They called me on 11-June and said "send us your passport as soon as possible so that we can issue you visa, also send us fresh medical, and visa photographs and fill the DS-160 form online as early as you can".

I was in Abu-Dhabi and Karachi Airport condition was critical. That's why I reached Karachi on 1-Jul. and sent passport and visa photographs on 1-July and Medical takes time that's why I sent it later on 9-Jul. And I already filled and submitted DS-160 form online in June (when I was in abu-dhabi).

Now after 6-June they updated the status date of my case again in 18-July. Means they have received my all documents.

Now I am waiting for their next action. As they already confirmed me in 11-June about issuing visa.

Passport (with visa on it) received today on 22-Aug-14. Also a sealed envalope . they said not to open it as only Americans will open it at U.S airport. So keep it safe.
