What to take to GP for immunizations

Is there a form that my fiance should take to his GP for his MMR, Tdap and influenza? We'd like to do this before his medical.

No form. Get your GP to print out an immunisation history from their system or write letter stating what you've had and take that to your medical.

* I-130/CR-1 visa by Direct Consular Filing in London
3rd May 2013 - Married in London
7th May 2013 - I-130 filed
4th June 2013 - NOA2 (approved)
16th July 2013 - Interview (approved)
30th July 2013 - POE San Francisco
29th August 2013 - 2 year green card arrived

* Removal of Conditions (RoC) via California Service Centre
1st May 2015 - 90 day RoC window opened
6th May 2015 - I-751 filed (delivered 8th May, cheque cashed 18th May)

They should have it all on their system, so when he goes he'll find out what he needs and he can request a printout of his vaccination history. If like mine the records are still half on paper they should be able to put them into the system then print them.

Handy tip... I found using the practice nurse much quicker to get an appointment with, more efficient than my GP and very helpful -she gave me the Td on the spot, (no charge) and typed up my vaccination history and left it for me to collect the next day.
