Please help about immunizations. Please give me a straight forward answer whether YES or NO if we still need to see a civil surgeon to transcribe our vaccination results into I-693..

The following are the results of her immunizations that was done in the Philippines on July 2013:

Tdap was given on 7/2013 and there's also a check mark under Insufficient Time Interval.

MMR was given on 7/2013 and there's a check mark under Completed Series

Varicella was marked VH under the Completed Series

Influenza was given on 7/2013 and there's also a check mark under Insufficient Time Interval.

The rest of the vaccines are all "not age appropriate".

Then at the bottom there's a check mark on the Vaccine History Incomplete and then there's also a check mark on the Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate (as Indicated Above)

Please help.. Thank you in advance!! :)

YES, you need a civil surgeon to transcribe the vaccination results into I-693 because that is what you submit for AOS.

I just came back from the doctor's office today. They said that all vaccinations with a note: Insufficient Time Interval must be given again... that is for TDAP and Influenza. Or if your civil surgeon is nice, s/he can waive it by checking the "blanket waiver" at the bottom.

Actually, we never submitted an I-693 form because we were adujsting from a K-1 visa and my husband had already completed a medical examination prior to arriving to the US.

Re. the I-693, the US website states that if you entered on a "K" or "V" non-immigrant visa and had a medical examination that was completed within the last twelve months then you do not need to submit the supplemental vaccination form. This is assuming that the doctor who did the medical examination overseas completed the vaccination record.

If they did not transcibe the records as part of the medical examination then yes, you will need to get a civil surgeon here to do it.

But again, we did NOT submit an 1-693 form with our AOS application.

I'm sure you are right, Sarah&Michel.

I just hate delays like RFE's. So I try to cover as much evidence and paperwork as possible...each case is different as I read through the forum.

Hopefully the end goal is a happy one!
