Correctly Completed I-864A Form

Hi Folks, can anyone provide me with a sample of a correctly completed I-864A Affidavit of Support Form (AOS). I desperately need this as we keep getting Checklist from NVC concerning same and we need a correctly completed from in order to ascertain where and why we seem to keep getting it wrong for NVC. All inputs will be greatly appreciated.

Its only 3 pages and self explanatory. Not too complicated of a form if you have a question ask it and we will help you as best

Florida4life, thanks for the response it is greatly appreciated. I am aware that the I-864A from is not complicated and consists of only three pages. However, I am haveing a hounderious time with NVC regarding same as each time our daughter respond to NVC's checklist concerning same she receives another and the last received on November 29, 2013 states the following:

General Instructions

[x] You will need to complete and submit the new form completely filled out (all pages required), which includes

the changes noted below. Submitting single pages of the form with the correction is not acceptable. In order

to make corrections you will need to download a new form at www.ImmigrantVisas.state.gov.

Part 1 Information on the Household Member

[x] This section is incomplete. Please answer all questions completely. (If N/A, please indicated in the

appropriate space) .

We did completed all sections of Part 1 on the I-864A from submitted to NVC, however, we did not state N/A in Section 3 of Part 1 which states: Place of Residence (if different from mailing address) and we are baffled that NVC would generate a checklist for this. We intend to submit a new I-864A form and just wanted to view a correctly completed one as it appears that we are too dumb to do so. The irony in all of this is that that we have supplied NVC with all the necessary documents to facilitate the completion of my case and obtain an interview date, however, we keep having the perennial problem to my wife's petition. And one would assume that if we were able to provide all the necessary documents etc to facilitate the closure of one case we would be capable of doing the similar of the other, however, for some strange reason NVC does not agree.

some how it says you didnt send all three pages back.

I would put the same address twice to put a end to that N/A
