My wife is here under K1 visa and now we are getting ready to file for AOS.. We live Illinois.. My question is, do we still need to see a civil surgeon or do we just include the vaccination summary sheet along with the I-485 form?

The following are the results of her immunizations that was done in the Philippines on July 2013:

Tdap was given on 7/2013 and there's also a check mark under Insufficient Time Interval.

MMR was given on 7/2013 and there's a check mark under Completed Series

Varicella was marked VH under the Completed Series

Influenza was given on 7/2013 and there's also a check mark under iInsufficient Time Interval.

The rest of the vaccines are all "not age appropriate".

Then at the bottom there's a check mark on the Vaccine History Incomplete and then there's also a check mark on the Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate (as Indicated Above)

Any information regarding this topic will be very much appreciated. Thanks in advance!

If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. .... http://www.uscis.gov/sites/default/files/files/form/i-693instr.pdf

dont forget to send a copy of ur DS-3025, just to make sure that if ur medical got lost in transfer you wont get an RFE coz it does happen to some adjusters... Good luck!
