Its asking if she lived anywhere else (another city, province, country) aside from the address that she wrote from the time she turned 16 years old until now (present).

If she lived in one address her whole life, she should answer "NO".

Put previous addresses if there were any since age 16 to now...if

she's been at present address all her life then put that address

My wife lives in the city since 2012 At her present address when she was 21 we rented an Apartment so many years until now but in the same time she visited her province where her parents still lived there unfortunately we have our own house where as she stay whereas her parents reside right across the street.

Good question.. but in our case a lil bit funny... ok my wife works in jordan for 2 years when we fill up the ds-230/ds-260 we never mensioned that she works in other country or she lived there for 2 years and i think she should need the PCC huh? But in NVC or USEM never ask the PCC from jordan.. maybe because we dont put in the list that shes out of the country for more than six months? Do you think guys They still trace that? or just becoz she change her status in the passport? i'am confused lol or NBI clearance is enough?

How about this question From date and to date:

Since born? until she gets married? Separate

From her parents
