Quick question guys. I went to my GP surgery to get my MMR and Tetanus vaccines done today - to return in a months time for the second MMR. What proof do I need that I have taken to the States when I apply for AOS and the rest? Do I get my GP/nurse to give me a print out of entries on their system or a short notes with the dates the vaccines were taken?

I would get something from the surgery (official) and take it with you to your medical in London and get them to fill out the forms there, this should go with your stuff in the sealed envelope you get along with your passport and visa. you then hand that all over at POE and hopefully it will end up with your AOS package.

Keep copies of whatever you get from the surgery.

Nobbie, I have done the medical and got my sealed envelop. I chose to do the vaccine at my GP surgery after to avoid paying unnecessary fee.

Had you done it at your GP before the medical, you could have been set for AOS with the immunization sheet done by Knightsbridge . Now you will have to pay a civil surgeon in the US to sign off on a new immunization form I-693. The civil surgeon will look at anything you bring in that looks official from a doctor office and evaluate your immunizations and recommend if you need more. Then he will put everything on the one form and sign off.

Just a note to your topic title....It is pertussis (not polio) that is the P in Tdap. For adults-

Tetanus, diptheria, pertussis

Mumps, measles, rubella

Varicella (or had chickenpox disease)

Influenza (Oct 1-Mar 31)

ooops! I should have paid the Knightsbridge medics to do them. Penny wise pound .....! Do you have a rough idea how much that would cost? Heading to Maryland/Delmar area

The trick is to find one who doesn't insist you have the entire medical again or a TB skin test. Civil Surgeons are in private practice and can charge you anything they wish. Call around and ask prices before you make an appointment. I've seen people charged several hundred and conned into doing a full medical again.
