Yes she can. There is no obligation in the USA to change names after marriage. It is a matter of personal choice.

September 10, 2009: Arrived for first meeting in-person!
June 17, 2010: Arrived for second in-person meeting and start of travel together to other areas of China!

Can she use her maiden name as her married name after we are married and fill out I-485 and EAD using her maiden name as married name?

She can use which ever she prefers. Has no effect on any of the process. Most people change the woman's last name as it is customary in their culture. Others do not for a variety of reasons. My wife has kept her maiden name and we have not had any problems during the immigration process. It does make international travel a bit easier as the GC and passport are in the same name, so no need to carry a copy of the marriage certificate. It is just a name. You are no more or less married if you change it or leave it the same.

On ours there is a space for "new names (if any)" which is the new legal name.

If you've created a new legal name by marriage, how does this affect the decision to move forward with the maiden name?

Not applicable to us, just relevant to this topic.

Isn't this a decision that needs to be finalized at the marriage license application stage? That's the way it was in our locale.

Name on our marriage license and marriage certificate is my name and residence at time of application and my wife's name and residence at time of application. So her name is the maiden name and her residence is in the US as that is where she was residing when we applied. There is no placed on our Marriage Certificate to change her name. It is assumed that her married name is my last name and her maiden name is the one listed on the marriage license. Of course we also we able to get certified copies of the Marriage Certificate the day of the wedding and received the original back in about three days. Each county and state are different.

Her maiden name will also be ONE of her legal names. If she decides to take the husband's last name that will also become ONE of her legal names. That is the reason for all names used in the past as that is a listing of all legal names you have gone by. Go to a lawyer and have a will drawn up and the person's legal name will be listed along with AKA (as known as) and all the other legal names that person has gone by. All are valid and needed just in case a document is in one of those "old" names.
