troubles with I-693A

Okay, we called most of the civil surgeons in our area (NJ, Essex County) but nobody is willing to transfer the DS-3025 to a I-693A without a full medical. I had my medical done in Germany on Sept. 12, 2006.The Surgeon only gave me a copy of the first page of the DS-3025 (sheet where all the vaccinations are listed). And now the Surgeons want me do to a full bloodwork again since i dont have no proof that i had it done in germany. Also thet want me to bring a filled out form I-693...

Anybody knows where i can download the I-693A????

Problem solved after calling about 15 civil surgeons we found one who is willing to do it for 25$

Good for you!! Most Civil Surgeons I called also tried to schedule a full exam. But when you finally find one that knows what you're talking about, it's worth the trouble.

Good! Encountered same problem @ redwood city so i searched more civil surgeons and I found one in SFO who made me pay 60 (expensive yes because of location) but didnt make me do everything again but just copied what I had.) . I cant imagine paying $165 for nothing... not to mention the pain is crazy!
