SLEC Vaccination Record and US AOS Civil Surgeon

From your experience, did SLEC fill out the DS-3025 vaccination form in a manner that seeing a civil surgeon is not necessary for the AOS in the US?

Well that is sort of an "IF" answer. SLEC does their work correctly so ... yes... sort of.

For the K-1 processing the I-485, according to the instructions, you only need to include a copy of the DS-3025 but at USCIS many times the adjudicator doesn't know their own rules and goes by the process for other types of visas and want to see the I-693 that was completed by a civil surgeon. IF you get a knowledgeable adjudicator then you will have no issue submitting the DS-3025, but IF you get an adjudicator lacking in knowledge they will give you an RFE for the I-693 (requiring a civil surgeon to transcribe the DS-3025 to the I-693).

I have made the suggestion that if one wants (live on the wild side) to send in the DS-3025 (per the instructions for the I-485) to staple a copy of the I-485 instructions page that states this information and highlight it so the adjudicator may become enlightened. ;)

I know of one person that has tried this recently but have not heard back yet as to whether it "flew" or not.

If you go by the I-485 instructions submitting the DS-3025 should be adequate each and every time.

We may just go ahead and go to the civil surgeon in the US anyways just to have a peace of mind. If we do that, am I correct in that all we need is page 1 and the vaccination page filled out and signed by this civil surgeon?

Yeah hopefully we can find someone willing to do just that in San Diego. Did you also submit page one of the I-693 or just the transcribed vaccination page?

The civil surgeon will transcribe the 3025 information to the I-693, seal it in an envelope for you to send in with your I-485, I don't know how many pages the civil surgeon puts in the envelope

Well you are required to fill out parts of page one, not the doctor, so if the doctor did everything and sealed it, then it must have just been the vaccination page.

The civil surgeon completed all the required information on his computer for the I-693 as we sat there, then printed two copies, one for us and one which was sealed in the envelope. He would not use the form we had already started to complete.
