Hi my name is gabriela and I'm preparing my I 485 and I want to know if I need to fill the DS-3025 vaccination record form, because I already did it in my country. but all the papers remained in the port of entry where the officer asked for my documentation.

If I have to do that somebody knows where I can do it cheaper in the east bay california? Thanks

You do not have to get another medical exam, and if you need missing vaccinations and/or the vaccinations transcribed by a civil surgeon, it would be onto an I-693 not DS-3025. That form only comes from the panel physicians in the foreign countries.
