My wife came to the US on a K1 Visa and we are now in the process of filing her AOS and are almost done but she needs to see a civil surgeon for them to fill out the vaccination record. We have the appointment tomorrow and I am hoping that someone can reply before then. There is nothing very clear about what to do at this appointment. We are taking the form I-693 with us and DS3025 and are hoping that they will know what to do. I have called lots of different civil surgeons and the one where she has the appointment seems to know what she is talking about and doing whereas the others were trying to charge me for another exam (my wife just had it back in August of this year). If anyone can share their knowledge and/or experience it would be greatly appreciated.

We had my husband's transcription (I-693 vaccination record only) done two weeks ago, and it was pretty straightforward. We took a copy of the DS-3025, as well his little childhood vaccination records as extra proof (not actually needed, they only really wanted the DS-3025), and of course his passport. The doctor fills out the I-693 -- you don't need to bring that. The doctor (well, nurse mostly) transcribed the vaccines onto the new form, filled out the basic info, and gave us a sealed packet to submit for AOS (as well as a copy of what they did, for our records). Cost was $50.

The only two confusing parts were: we decided to go ahead and get his flu shot, since it's flu season now and that is required for the AOS during flu season, so that was an extra $25. The more confusing part was the TDaP: my husband had had a DT booster just six months ago in Finland, and the Finnish civil surgeon thought that was enough -- but the doctor here in the USA said he absolutely needed the P (Pertussis) booster in addition to DT, so he had to get a whole fresh TDaP booster, which was another $75. Oh well -- at least we really know everything is current now!

N400 filed September 14, 2023, same day acceptance and Biometric Reuse notice, Interview on 2/13/24: Passed and same day oath. ALL DONE WITH USCIS.

They gave you a DS-3025 in Juarez? Was it blank, or they actually did vaccinations for a K-1 and filled it out? I am just curious because the clinics in Juarez have had the policy of not bothering to deal with vaccinations or the DS-3025 for K-1 applicants at all. They would either give a blank DS-3025 or none at all.

You need to get any missing required vaccinations and have page 1 and the vaccinations page of the I-693 filled out by the civil surgeon. They should give you a copy and seal the original in an envelope for you to include with your AOS paperwork. No medical is needed, if your K-1 medical was within a year before filing for AOS. They may try to give you one anyway, or do parts of one, like TB testing and blood work. None of that is actually required since you already had the medical exam recently. Many insist on the medical, parts of a medical or to charge you for one anyway. Why? Because they can. You have to call around to find one that won't try and rip you off. There is a long-standing thread here on VJ about civil surgeons that won't rip you off.

They didn't give us DS 3025 in Juarez. I read and heard about the form and got it from USCIS and printed it out. My wife has her appointment now so we will see what happens.

It has to be filled out by the panel physician at the K-1 medical exam or it does nothing for you. It is not needed to get the vaccinations and I-693 filled out by a civil surgeon in the US.
