1.) I work as an Independent Contractor. Sometimes my title is Brand Ambassador, sometimes it is Promotional Model. I do NOT have one specific employer so I am having issues as to what I should write down for question # 7. Worse-case scenario, should I just choose one of the ones that actually does take out taxes from my paycheck? Most of them, I do a W9 for, and they don't extract taxes.

2.) What should I put down for question 11? I have a fiancee that is coming hear with no job lined up. I intend to handle EVERYTHING at first, but I can't put that in the explanation lines provided. What is a commonly accepted and expected in that part?

***** I am from the USA, She is from Japan

Why not just put business of marketing and that you are self-employed?

The example form shows the answer for someone using the form for a K-1 > http://www.visajourney.com/examples/Form-I-134.pdf

Since the K-1 visa actually has immigrant intent, that question is not applicable for a K-1 applicant, which is why the example form shows to check neither box and put N/A. However, if it makes you feel more comfortable, you could also check 'Intend' and still put something like, "K-1 visa process for permanent residence ( provide all expenses as necessary)" on the line.

Thanks for the reply

That was the confusion for Question #11.

For Question #7, there are 3 slots provided. Would I just put down answers like: Marketing, Self-Employed and Various where it asks for the address? That address line would look weird as then they make ask for an attachment or something. It may be better to just put down an address of a company that I work for frequently this year.

Any other opinions

You are your own 'employer'. Your address could go there. Some self-employed have a separate business address they would list there.

I also have the same question. I do not work for a company. I work for an individual. What do I put on question # 7?

Are you self-employed, or is the individual your employer? If you work as their employee, then they would be listed there. If you are an independent contractor and self-employed then you list your business type/occupation, self-employed and your own address as sole proprietor.
