First of I guess you will be filing and had not yet filed.

1. Born US citizens do not have A-Number (only naturalized citizens have an A-Number), so leave it blank.

2. Include your child and answer the "NO" to the question 6 "Is child applying with you?".

3. Yes, you can get 2 or 3 affidavits from US Citizens. This is not a great evidence but an additional evidence. No need to get it notarized.

4. Make sure that you mention about your stay with in-laws in the covering letter. Also get a letter from in-laws that you stayed with them for the period and attach. This is important evidence.

I don't have to apply the my child i think no because he is born in usa and he is usa citizen so there is no need?

What wording should i use for my parents in law letter we need to notarized that

You must include all children and answer appropriately the two questions.

Use this link and go down to the I-751 example forms and you have one for the affidavit, which you can use.

It said information about children and it is part 4 not 3 so that you need to give your children information as he's your child nevertheless he's USC. They wants you all family member information. Answering "No" to question "is the child applying with you" means he doesn't need to apply with you since he has his own status already.

Ok, you need the following important evidence. Others may be added as you desire.

1. addresses where you lived attached immediately to the I-751 application.

2. copy of both front and back of the Green card.

3. copy of birth certificate of your son.

4. full copies of joint bank statement - one per each quarter covering the full period of 2 years or at least from the time you made those joint.

5. IRS tax transcripts or full tax returns including w-2s for 2011 & 2012. Did you not file in 2011?

6. copy of joint car insurance cards, cell phones and any others that are joint. If some utilities are in one name and other in other name, they are good. attach those too.

7. Joint lease is very important - don't you have those for the 2 years?

8. Rest are additional evidence and may or may not be attached.
