Recommended Civil Surgeons in Chicago-Help!!!

Hello All,

I am desperately looking for a good Civil surgeon in the Chicago area to transcribe my vaccination records from my DS-3025 to my I-693, can anyone recommend one who won't make me have a medical, TB skin test or blood test for Syphillis?

Trying to explain to them that the USCIS have my blood results is like talking to a brick wall!

Any help would be greatly appreciated.

Tried the Immediate Care one listed on the 'Civil Surgeons who don't rip you off' and guess what?

Tried to charge me for a full medical, blood tests and a TB skin test. $300. Left in a hurry!

3. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

a. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or as a

b. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and

c. You received a medical examination prior to admission, then:

(1) You are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.

Yeah, came in on a K-1.

Thanks for that: I saw that info, which is why I didn't believe the BS they tried to tell me. I have a completed DS-3025 but worried that if i file just that i'll get an RFE, as I didn't have a flu shot because it wasn't flu season...

That shouldn't be a problem. If you were current on the date of your medical then that is enough. If you have the waiver marked "not flu season" then the flu shot is waived no matter if it is flu season when the adjudicator picks up your case. If you went to a CS now, then you are getting a "new" date to go by which is in flu season. Save the hassle and money and use the DS-3025 as your immunization record as long as everything was complete as of that date.

Also, if you haven't figured it out yet, an RFE is not a big deal. It's not delaying getting to be with your loved one. You are together now and an RFE isn't going to change your day to day life that much if you have a week or two delay getting a greencard.

To whom need a vaccination transcription for the AOS in Dupage County/ Aurora(Chicago suburbs):

After calling several civil surgeons from the uscis list I found one in Aurora who only charged me 80 dollars( others were asking around $200) and did a pretty honest straightforward clean job with me. The civil surgeon in question is Alberto Silva, actually i never met him, it was his secretary/nurse who dealt with me. Lorena speaks English and Spanish, ask for her when you call. They open on Saturdays till 1 pm.
