I've read all the documents provided by the Embassy (They don't send out a information packet in Kiev, everything is in PDF)

Here's my interview check list. Am I missing anything? Thanks in advance!

  • Original Birth Certificate

  • Ukraine Passport

  • Police Certificate

  • Medical exam certificate

  • Marriage Certificate

  • Divorce Certificate

  • 3 Passport Photographs

  • Form KEV-1

  • Form DS-156

  • Form DS-160 (online)

  • Evidence of Support

    • Form I-134 (Original!)

    • 2012 Tax Return 1040

    • Employment Letter

    • Last two pay stubs

  • Proof of relationship

    • Skype logs

    • Airline tickets

    • Engagement ring receipt

    • Hotel receipts
    • Pictures

You need two more letters of intent, dated for this period in time (for you and your fiancee). Also, I don't believe that you are required to complete ds-156. Tax return is good, but they prefer tax transcripts from the IRS. Maybe someone else can chime in on the ds-156, but I believe that they replaced it with the ds-160.

Thanks Hin-d, the only reference I found to DS-156 is on visajourney.com - Kiev Embassy. I can't find it in the Embassy instructions so I'll just remove it.

Good call on the letters of intent. I remember reading that somewhere! Even if they don't require it , it certainly won't hurt.

Thank you! Good luck to you, too! Don't forget to take the bank receipt, for the visa payment, with you to the consulate...and DS-160 confirmation page(I'm sure you knew about the confirmation, but you didn't list the receipt).

The DS-160 confirmation page and everything listed on the KEV-1 checklist is all you need. The updated the Letter of Intent is also a great idea. Just be sure to provide all the supporting documentation for I-134 Affidavit of Support Form as well.
