Medical document question

Can anyone tell me which document from the medical review is necessary to file with the Adjustment of Status paperwork?

Any other info or insight regarding this question would be greatly appreciated!

The packet you received from the Civil Surgeon whom she saw in the US. It will be sealed by them, you will present your medical from the CS in the Ukraine to the US CS. Mail the new US CS package with your filing. See the guide for AOS. I693 form Report of Medical Examination. See the VJ link and check the USCXIS site, for some reason I could not attach the links. If you are married and she is on our medical it will save you some money on the exam / shot part.

I read your timeline... My fiancee' (now wife) went through Kiev... The I-693 medical is done before the K-1 interview and "possible" K-1 visa issuance.

Assuming you and your fiancee' sucessfully complete all of the required steps PRIOR to her AOS, then she should only need to include a copy of her DS-3025 vaccination record with her AOS package, but that is assuming that the "sealed" case package (that she will carry to the US) remains sealed while in her posession, contains all of the required papers from the consulate in Kiev, and that it doesn't get lost or stolen and that its contents are not altered, and that it doesn't get opened until it ends up in the correct hands upon her POE arrival into the US.
