WTF Extra form that goes along with affidavit

When you get to the Embassy part of your K1 then they can ask what they want and all Embassies are different as Philippine rarely accept a co sponsor but many other Embassies do. So not a clear picture of what is the normal. Sorry but if you have evidence than just abide and get original signatures and express mail. Way better than being denied at embassy.

Just very curious that my friend who was approved a little over a month ago (at the CR embassy) did not have this in his packet. Would the embassy change what's required on a case to case basis?

Frustrating, but luckily our co-sponsor in the states and my best friend are AMAZING and already have this "important" doc on the way. Of course I am happy to do it now and hopefully smooth sailing from here on out!

Sooooo frustrated!!! We just picked up packet #3 from the Embassy in Costa Rica. MAJOR SURPRISE with an additional document that is supposed to go along with the affidavit that looks something like this

"IMPORTANT"

This document must read and signed by persons wishing to submit an affidavit of support on behalf of an alien applying for an immigrant visa. A signed copy of this document must be attached to each copy of any affidavit of support submitted on behalf of an applicant.

Then it says stuff about the Social Security Act, and Food Stamp Act and who and how one is eligible to apply for them, etc.

At the bottom:

I, _____________ residing at ____________, acknowledge that i have read the above and am aware of my responsibilities as an immigrant sponsor under the Social Security Act, as amended, and the Food Stamp Act, as amended. The statement I am submitting on behalf of the following persons.:

Then there is a space to put names, sex, age, country of birth, married or single, relationship to sponsor.

I have never seen or heard of this document before... Do any other consulates require it? Did something in one of those acts just suddenly change? Such a HUGE SETBACK as our co-sponsor has already sent the signed affidavit.

If we do have to turn this in, the BIG QUESTION does this form has to have the original signature as well??? I have kept cool and collected basically the whole time, but now I'm pissed.

Too bad different embassy have different rules. Something I have never really understood as they are all American Embassies?

Called the embassy and they said if it was included in the packet, it needs to be completed and turned in with an original signature. Speechless now because another friend had an interview a little over a month ago and it was NOT included in his packet.

Now that I had to learn the hard way, how can we get it on the forum so people are aware that this could happen to them too?
