Does anyone know if they make you have all your vaccinations for your medical in Juarez? My fiancé is missing one of his DPT shots. He had the first two when he was little but never got the third one. And he doesn't have the flu shot. Are they going to make him get those shots or should he get them before

They do not tend to give vaccinations for K-1 applicants. They don't check records, and they don't give any shots or provide a properly filled out DS-3025. Unless something has changed very recently, that will most likely be the case for him.

If he gets any of the required vaccinations someplace there in Mexico, then he will need to bring those records with him to the US. You will need to find a civil surgeon in the US to get any missing vaccinations and have the ones he does have records for marked off on an I-693 to submit with your AOS paperwork later.
