I thought I was set, just gather all the forms after marriage and done.

But naturally this is not the case:

I only have a copy of the medical form that says DS3025 on it from my medical. It is a vaccination sheet, but it does not have the illuster I-693A number on it anywhere. Do I use the DS3025 instead? or is that worthless?

I am really up against the wall here, all this paperwork is making me sick once again. I can fill out every other file, but not that one since it needs to be signed by a doctor. Why does it all have to be so f*ckin' complicated?! I hope there is someone from the netherlands who went trough this, because I am at loss...

Your vaccination record, DS-3025, needs to be transposed by a US Civil Surgeon to an I-693A. There have been many posts about this recently.... do a search in the AOS forum and see what people have had to say... there have been many varying experiences....

It's not as bad as it seems, Justice. Try to relax a bit.

You simply need the information on the DS-3025 transferred onto the form I-693A by a Civil Surgeon. The USCIS website has a list for all the Civil Surgeons in your area. Call as many as you need to find one that will provide this service. If a complete medical was done within the past year, you do not need to pay for another, despite what some of the CS offices may tell you.

12/30/2008: Overnighted I-751 package to VSC

01/06/2009: Check cashed

01/06/2009: NOA (arrived 01/09/09)

01/23/2009: Biometrics appt letter received

01/31/2009: Biometrics scheduled

05/20/2009: APPROVED

06/23/2009: 10 Year Green Card arrived

Does your DS-3025 show that all vaccinations have either been had or are medically inappropriate and should be waived? If so, many people have used the DS-3025 and have not needed an I-693A. Other people have been asked to get an I-693A anyway. Don't know what the Dallas office has been doing.
