Just a couple concerns I had when filling this in. For one, is my spouse considered as someone who is dependent on me? Or is that completely separate from this. Also, how and where can you put in for a co-sponsor if needed?

Timeline

You and the co-sponsor fill out one each. Supporting docs include tax transcripts, letter from employer, recent pay stubs, etc.

At the top of the page there is a link that says "example forms".

Also I'd recommend you go to uscis.gov and download the form & it's instructions. I just did that and filled mine out with no issue.

2014-02-23: Vegas wedding - Elvis style!

 

Adjustment of Status
2014-03-28: Mailed AOS, EAD, AP
2014-06-16: EAD/AP approved
2015-02-20: AOS approved

 

Removal of Conditions

2016-12-05: Mailed I-751

2016-12-07: NOA

2018-05-19: Approved

Your spouse does count as dependent. Co -sponsors fill out their own forms as their data is different from yours.

05/21/2015 - Interview - Approved

If you are filling out an I-134 for a K-1 visa, then you should not have a spouse. The foreign fiance(e) is not listed in #8, if that is what you were trying to ask. http://www.visajourney.com/examples/Form-I-134.pdf
