Name to use for AOS and Medical Transcription Questions

Hello Everyone!

I'm about to file my Adjustment of Status (AOS) and not sure what name to use... my maiden name or my husband's name. We just got recently married and I decided to take my husband's last name. All our K-1 documents and Social Security card contain my maiden name. Will it confuse the USCIS if i append my husband's last name?

Another question... regarding the I-693 (Report of Medical Examination and Vaccination Record), part 1B (Syphilis), 1C, and parts 2-6: are those necessary to fill-out by the civil surgeon and include in the packet? Or just the information in part 1 and Vaccination Record are necessary?

Use your married name. Since you are taking your husband's last name your forms should have your given name and then your new last name. USCIS will issue your greencard in your new name. If your SS card has your maiden name, then sounds like you have to go back to the SSA with your marriage cert and change your name on your card to your married name.

Yes. Any transcription of vaccinations to the 693 must be done by a civil surgeon recognized by the USCIS. Use the locator HERE. The trick is to find one in your area that will only do transcribing. Most civil surgeons want to charge/do the whole medical exam all over again. Good luck.

Thanks so much for all the tips. We just went to the civil surgeon and everything has been taken care of. We're now just waiting for our marriage registration from the registrar's office.
