A Second Visa Fee ??

I was approved yesterday at 5pm so of course since then I've been doing research on what's next and what to expect.

I read on one of the links here that I'll have to a pay a $400 visa fee "if it wasn't already paid for you in the USA" .....

What does that mean ? Do we pay a second fee or is this the $340 we initially sent..?

I just want to know how much I should start saving up for the rest of this process
