How does the sponsor need to count the household?

He is married, so it's only him and his wife.

But a sponsor needs to be able to provide for himself, both me and my fiance. (That's what the embassy told me)

So is his household 4 then?

The co-sponsor needs to make enough for his household plus the immigrant. The US citizen fiance of the foreign beneficiary doesn't count. So in your case, assuming the co-sponsor has no kids, it should be a household of 3 (him, his wife and the immigrant).
