starting AOS

So, I finally got all the documents I need to AOS.

I'm a F-1 student, married to USC, and AOSing.

I have a couple of questions:

1) I understand that I need to send everything (i-130, i-485, EAD) to chicago lockbox, is that correct? The forms are confusing me since they're saying to file to the local uscis office.

2) Do I need to print and include form pages that are instructions? i.e there are no fillable fields on them.

Correct! Everything gets mailed together to the Chicago lockbox.

No, only the part you fill in is necessary, which of course usually starts with your name!

What should I write? I currently live in LA, but I will obviously submit all of this to chicago lockbox. What should I write? Should I write that I will apply for AOS in my local california USCIS office? Or what? I'm confused.

I had to look for information on that question too when I was filing out mine. I forget where I found it, but the answer was to put in the office where you would be having your AOS interview (hope this is correct as it's what I did).
