A last question.......on the B1/B2 visa, is the 6 month limit per stay, or per calendar year? I visited the USA in September/start of October for 3 weeks, so need to know because if it is per calendar year then I'll need to plan a shorter stay.

Unless you are given a mandatory return date (I-94), you are allowed 6 months per visit. But keep in mind that the rule of thumb is that you should be spending more time out of the US than in it. They will have access to your previous trips and can see how much time you are spending in the states. Whether or not your previous 3 week stay will matter in how long of a stay you are given this time is completely up to the CBP Officer. I would not advise asking for any longer than 6 months.

It is UP to 6 months, it is at the discretion of the CBP officer the amount of time you receive on your I-94, or if he allows you to enter. As everyone stated, that without a return ticket you will be denied entry at the airport.

Get an open return ticket. I've done this plenty of times. You get a return trip on a date that it's likely you'll leave. If you change your mind, you're allowed to do so. Most airlines will charge a fee to change the date, but if you can afford an up to six month vacation, you can afford this fee as well.
