hi all....just a little confused here.....can we have any doctor fill out the 1693A..or do we have to pick out someone on the list from the immigration site?....cause i just called those ones and they all want us to have a complete physical...which we already had....ugh!!!!...now there is me and my 2 sons..18 and 20....gets a little expensive...sooo.....can i just call up someone my fiance knows or not?.....thanks all

You need to choose from the designated civil surgeons listed on USCIS's web page.

It has to be from a doctor on the list... Any old MD will not cut it...

It seems to be a little scam with these civil surgeons. Although you have already done the physical, they refuse to only transfer the vaccination record, and insist on a complete exam. I was lucky enough to find a civil surgeon very close to my home who transferred the vaccinations for only $25.00.

i wish you have mentioned this a few months back. we went to my doctor (just recently became my doctor because our old family doc retired...darn she was the best ever...anyways) who also happens to be a Civil Surgeon listed on USCIS however she charges a flat fee of $150 (cash only) for the I-693. i tried to explain to her that we only need one more vaccination and to transfer the information for the other vaccines...and that we only need I-693A...and that he doesn't need another medical check since he just had one a couple of months back...we're on K1...blah, blah, blah.... anyways, not only that she did not budge but she also said that she is not the kind who does shortcuts and she said...since she's indian-american and was an immigrant and that i am too that i should know we can not do shortcuts on the immigration process or else it will not turn our right...pretty much making me feel like i'm trying to do something illegal. oh yeah, she won't accept the xray results from St. Luke medical (from Manila)...so we had to get another xray done ($65) because she won't complete the I-693 form without it. the imaging center won't read his xray film either because they only read xrays from their facility. anyways....$215 dollars later...and a lot of calling back and forth...and driving back and forth...we were finally handed our sealed envelope.

i hope there was a better system to assess the medical for the immigration when you're in the states.

i just wanted to get that off my chest. (i've switched med insurance also...and will have another doctor. )

I had the same experience as well all the civil surgeons I called wanted to redo the medical exam and all wanted over $200 dollars for it. I called this one place and actually started arguing with the receptionist, I told her we only wanted I-693A done, he had even had his vaccines done at the local health center for free, I finally told her to treat the case as a refugee/asylee and she said oh! you just need the vaccination supplement, ok that will be $50. The day we went to his appt. they tried getting him to sign the medical exam sheets, I went up to the receptionist and told her this is an appt. for the vaccination supplement only. She apologized, but when she got my husband back there she asked him to give her $20 more dollars for I don't know what, he said no, my wife has the money(not true), he told her to explain what they needed the other $20 for. She said "never mind," when he came out she said "$50.00, don't worry we didn't charge for nothing else." She gave us the vaccination supplement and a brown sealed envelope with a duplicate vaccination supplement inside, which we sent in with AOS package, still sealed.....

I haven't gone yet but I called last week and they said 15 bucks for the vaccination record, I called this week to see when I could come in and they said "swing by and we'll fill it at any time" and also said it's 10 bucks.

What a rip off!! But best to get your applications in ASAP, especially considering your son is 20.

We went through the same thing. We contacted the doctor TracyLuis used (see Devilette's links) and paid $10 to have our records transposed from the DS3025 to the I-693A.
