problem in birth certificate

Timeline

I was petitioned with my mother' s surname because my parents are not married. But I'm worried now because we found out last month that my birth certificate has an annotation already it says that "The surname of the child is hereby changed from Agustin Guero. The child shall be known as Ann Agustin Guero pursuant to RA9255." But the original surname 'Agustin is still there it's just, there's an annotation. Will it be a problem in proof of documents or for the interview in embassy?

So the NSO annotated your Birth Certificate without your knowledge? Has anyone in your family requested the annotation? I requested an annotation on my BC due to my mother's name that had a missing letter but we went to the city hall to request the changed and after that we made a request at the NSO office and paid the fee.

I do not know that NSO is now updating BC's under pursuant of RA9255 law without the owner's knowledge and FREE! ;-)

October 17, 2013 →Transfered to Nebraska Service Center
November 19, 2013 → Case Approved
November 22, 2013 → Case Sent to DoS
December 16, 2013 → NVC Received the Case
January 9, 2014 → Got NVC number via phone
January 16, 2014 → AOS Paid
January 28, 2014 - IV Paid
February -- Joint Sponsor back-out =(
April 4, 2014 - Found New Joint Sponsor =)
April 10, 2014 → Sent AOS package and IV documents
April 17, 2014 --AOS package and IV documents entered the system
May 2, 2014 → RFE for AOS
June 2, 2014 → Sent new AOS package
June 6, 2014 → AOS package entered the system
July 8, 2014 →NVC Case Complete
August 4, 2014 → Interview ( APPROVED!) under AP waiting for HK Police Clearance Certificate.

Even the employees in lcr told us that it's the school where I graduated high school which is responsible with the annotation, because they want me to graduate with my father's surname but my father refused. Because the year in my annotation is the same with the year of my graduation. They even advice us to file a complaint to those people who are responsible with the annotation. :)

Your High School is legit to file the RA 9255 but it should have your father's permission which he declined. I was wondering how your HS did the annotation because requirements for them to use (AUSF) Affidavit to Use the Surname of Father are two handwritten documents such as ;

1. Employment records;

2. SSS/GSIS records;

3. Insurance;

4. Certification of membership in any

organization;

5. Statement of Assets and Liabilities; or

6. Income Tax return (ITR)

File a complain! LoL
