How to answer question 9 & 37 on DS230 Form

Hello!

I am am trying to filled out the DS230 form so i can send it to the embassy to schedule my interview. I already look at the sample example here visajourney and that's basically for single women/people. How should a divorced woman answer questions #9 and #37.

# 9. Marital status (Single,Married,widowed,divorce,separated)

Including my present marriage,i have been married____times.

# 37. Other Names Used or Aliases (If married woman, give maiden name)

A&J K1 visa Journey

Sent I-129F packet Sat-June 8,2013.

USCIS received 1-I29F Packet Wed-June 12,2013.

NOA1 Notification (email & text msg) and Notice Date Fri-June 14,2013.

Awaiting NOA1 hard copy in mail.................................

Alien Registration Number was changed relating to your I129F, PETITION FOR FIANCE(E) On Thurs-June 20, 2013

NOA1 hard copy in the mail Fri-June 21,2013

I129F, PETITION FOR FIANCE(E) transferred from VSC to TSC On Fri-August 9, 2013

I129F,Petition for FIANCE(E) transfer and now being process at TSC Thur-August 15,2013

Receive Notice of Action Transfer (NOA-T) on Mon-August 19,2013

NOA2 Notification (Email) Wed-September 18,2013

NOA2 hard copy in the mail Tues-September 24,2013

I129F is shipped from USCIS to NVC Thurs-September 26,2013

NVC received approved I129F and assign case number Fri-October 4,2013

NVC shipped I129F to Embassy Wed-October 23,2013

Timeline

Filing a DS230 or DS260 form depends on the embassy/consulate. As far as Mumbai consulate is concerned, DS260 is yet to be introduced there. They are advising to file DS230 form only.

U.S. Embassy Kingston will schedule your K1 visa appointment when we receive the I-129F petition from NVC and the DS-230 form has been completed and returned. If you have not yet received instructions to complete the DS-230 form, please fill it out and email it to kingstoniv@state.gov or mail to: Attn: Immigrant Visa Unit, 142 Old Hope Rd., Kingston 6.

Please fill out the forms below prior to your K1 interview.

Yes Sureshot53, the ds-260 was completed and the Embassy said please complete ds-230 part 1 and email back to them.

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

Timeline

CEAC status date changed again today, called DOS, no interview scheduled, I wonder why the date was changed, it changed when they received the packet 3 on 10/04 so what is it this time if not packet 4 being ready to be sent.

I WILL ADVISE YOU TO CALL AND CHECK AGAIN, MY STATUS DATE CHANGE TO DAY AND I CALL DOS, AND THEY TOLD ME I HAVE A INTERVIEW DATE, SO I WAITED 20 MIN AND CALL BACK TO COMFIRM, AND I WAS TOLD I DONT HAVE A INTERVIEW DATE, SO I ASK HOW COULD THAT BE WHEN 20 MIN AGO I WAS TOLD DIFFRENT, THEN HE CHECK AGAIN AND COMFIRMED THAT I DO HAVE A INTERVIEW DATE,
