Document Delivery Address Question - ustraveldocs website

Hi,

I am little confused with what to do here in my situation. I applied for my wife visa under her passport with her father's name and last name but when I went to get PCC done at Ahmedabad Passport Office they required to change middle and last name to be husband's name so we did that and got new passport.

Now, I am trying to sign up for Document Delivery Address on ustraveldocs website where they ask for passport number, first name, last name etc. I am not sure which information to enter there. Should I sign up with old passport information(which is used to apply for her visa process) or new passport information?

Please let me know if someone have answer to this...
