Plz help .....Filling I-130 for my parents

hello all, im trying to complete form I-130 for my parent. But i got stuck at number 17, Part C : Information about your relative . As my understading , my name should be on the list of " spouse and children of your relative ". However, i got a name change when i became citizen . So which name should i put in that list ? my old name or my new name ?

my second question is Can i send all my parents forms and stuff in the same package with 1 check or do i need to seperate them into 2 packages ?

P/s: i tried to call 1800 number of USCIS but they can not give me an answer for it.

i would suggest use the name that you are using for Legal Documents like passport and Green Card/Citizenship. I hope it help a little

Did you just change your last name to match your husband's last name? Did you change your first name?

Yes! And do list "any" other name(s) you have used in the past (that is NOT the same as your LEGAL name at the moment, the one listed on your Naturalization Certificate)
