Plz help .....Filling I-130 for my parents

hello all, im trying to complete form I-130 for my parent. But i got stuck at number 17, Part C : Information about your relative . As my understading , my name should be on the list of " spouse and children of your relative ". However, i got a name change when i became citizen . So which name should i put in that list ? my old name or my new name ?

my second question is Can i send all my parents forms and stuff in the same package with 1 check or do i need to seperate them into 2 packages ?

P/s: i tried to call 1800 number of USCIS but they can not give me an answer for it.

Do you mean Use my new name as it was changed during citizenship ?

i will use seperate package for my mom and dad , but can i put them together in same envelop when i mail it ?

what do you mean by single name ? i changed my name during my citizenship not during marriage .

You will fill out two sets of forms send a separate check for each,

birth-certificate of the name they gave you & copy of proof of name

change at naturalization, also marriage certificate if one exists along

with detailed explanation
