DS156 Quick question

Hi, Im filling in DS156 and have a quick question: Where it says 14. National Identification Number (optional)- do I put my UK National Insurance number? It's unique to you...but not offically an ID number! Thanks a lot...Im going crazy with stress filling this stuff in. :help:

Also...

For 2, 3 and 4- the passport issue City and State...I just sent it away via the post office, I didnt go to an office, so do I put N/A and just England as the country?

And 29 Have I been to the USA, there is NO room to write the trips...so a seperate sheet attached with from / too dates would be what to do?

The UK has just implemented a national ID system, so at this point probably no one has ID numbers yet, the national insurance ID is not the national ID, just like in the USA a person's SSN is not a national ID number. So put "none" there.

Where did you mail your passport application to, in my case in the USA I mailed my passport application to the state department in Boston Mass. If you don't know then just fill in England as country.

For 29 You attach a sheet listing your trips, and at the top of the sheet, state "Question 29" and on line 29 write "See attached"

As for the passport...I think it went to the main Liverpool place...but the envelope I got it in says Penrtih. So maybe best to just leave it N/A? Thanks!
