I485 help: "Application type" "Last entry" and "Current USCIS Status"

I'm working on my I-485. I was wondering, as a Canadian who last entered the US as a tourist, what do I put under the "Current USCIS Status" box, and expiration? As a Canadian we're not given I-94s and can enter the US with a simple passport.

Also, for Part 2, under "I am applying for an adjustment to permanent resident status because:", which box do I tick off?

I entered the US as a tourist and stayed here for a few years and recently got married to a US citizen. I'm finally working on my AOS I130/I485 paperwork. The wording is kind of confusing but some sample documents I looked at for a similar case had the A box ticked off.

And what do I put for Place of Last Entry Into the United States? I entered US customs at the Pierre Elliot Trudeau Airport in Montreal and flew into the US. I don't remember if I flew directly to Los Angeles where I currently am or if I stopped over somewhere like Chicago.
