Anybody have the name of a reasonable civil surgeon in Wisconsin?

The two surgeons I called both require a full physical at a price of $700 + Immunizations. They say the immunization records from other countries are unreliable so the shots will need to be done here.. They will not simply transfer the data. Seems a little steep to me. Anybody have better luck.?

I'll suggest to keep looking, if you can do it in another state, it might be better. a 250-350 would be more fair fee.

These surgeons are just trying to get your $$, of course, they probably have a policy of not accepting vaccination records from other countries.

The medical she just got is good for one year. She doesn't need another for the AOS. We did the follow up shots at our local health clinic where they changed me 23 dollars for three shots. We asked them for a shot card and to stamp and sign the form. That was submitted at our AOS meeting. We were asked no questions about this subject at all.

We went to Concentra on the east side of Madison, and they were listed as Civil surgeons on the website. It was two years ago, but they transferred everything just fine and gave my spouse a few of the second doses needed. I would recommend them, but call them first to make sure they are still doing that.

Looks like Madison has a bunch. Too bad it's a 5 hour ride each way.

I tried explaining the whole one year rule. It fell on deaf ears I'm afraid. they know we need it and with only two docs in the area, not much incentive for them to play nice. Just steams me that we paid for this just a few months ago and now need to pay again.

Guess I need to start looking at the balance between a day off work against paying extortion.

The medical she has will suffice for the AOS if its not more than a year old. You do not need another one.

Yes.

Actually the medical report is part of her file right now. It will follow her to the AOS meeting. That's some of what is in the mystery envelope she turned in at the POE.

My medical wasn't one year old either. Still we went to our local county health clinic were I got 3 follow up shots and my husband had them sign and stamp the form. We turned that in at the AOS meeting but the officer simply added it to the file and never asked one question about the subject.

It all depends on how the DR in Thailand marked the medical as complete or incomplete. If it is incomplete then by reading the guides you would need to see the civil surgeon and get the missing vaccinations here it says:

I-693, Medical Examination of Aliens Seeking Adjustment of Status.I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy).

If you are missing the vaccinations you can go to any old DR and get the record you had that done and bring the record to the civil surgeon.
