Hello vjyers about 2 weeks ago we received the letter from USCIS requesting for my transcribed vaccination sheet and this past week we finally put it together and ready to send it in now my question is where do we send the RFE response as there is no mailing instructions in the letter aside from the note that says we need to send the letter back along with the requested form and there is only one address in the letter and that is the Field office here in Kansas so should we send it back to the field office here in Kansas or the National Benefits Center where it was forwarded from Chicago lockbox?

We just wanted to make sure it goes to the right office and get it done with as soon as possible.

Thank you so much for your help.

Since that is the only address provided, that is where I would suggest that you send your RFE response. Be sure to make a duplicate copy of everything that you send to USCIS.

Good luck.

thanks a lot A&B our very concern is that this document is sealed and even if we have a copy of it they need the sealed one from the civil surgeon thats why we really wanted to make sure. Thanks for your suggestion we are considering it and will just do a certified mail to be sure. Thanks a lot :)

Sounds good. The Seattle-based CS that we dealt with for my wife and stepson's immigration exams gave us a copy of the completed reports (for our records) that were sealed in the envelope. Assuring mail delivery with signature confirmation is a prudent move.

Good luck.

Hi there. I just came across this entry, which I see was written at the end of September. I am actually in the exact position you were in last year (RFE for vaccination documents; no instructions stating where to mail the requested info.) and just wanted to find out how your case turned out. Did you end up mailing your vaccination documents to the local field office or the national one? Any information or advice would be greatly appreciated!
