report of marriage form pls help....

hello everyone after 2 years of marriage, we are going to file my report of marriage but confuse with REVISED JUNE 2013 report of marriage form....

question number 9 CIVIL STATUS the choices are SINGLE, DIVORCE, WIDOW/WIDOWER

why there is no married in the choices..

and at the bottom of the form

EMBASSY/CONSULATE OF THE REPUBLIC OF THE PHILIPPINES

do I need to fill them up also???

does it mean under wife NAME -last name and first name would be my name before I get married? and number 8 question RESIDENCE is Philippines where I lived before I fly in the US???

and how about the bottom of the form do I need to fill that up too?

We just completed our ROM. As noted above the civil status is before the wedding. Joan listed her maiden name but was told to list Honolulu Hawaii as her residence. The bottom of the form is necessary (parents, etc. and the wedding details). Include the place where you sign the form in the "at" space (we forgot and the consulate filled it in for us). You will need 4 copies with original signatures (not photocopies). You do not need to have the forms notarized if you appear in person. You can pay in cash if you appear in person. Include a self-addressed stamped priority mail envelope if you want your copy mailed back to you. We had our copy within a week.

ROC (I-751) sent to CSC via USPS Express Mail 8/31/15

ROC check cashed 9/4/15

ROC Biometrics 10/1/15

ROC Approval 4/6/16 (waiting for actual card)

Permanent Green Card Arrived 4/14/16
Naturalization Interview 2/22/17 APPROVED!

Oath Ceremony 3/21/17--Joan is a US Citizen!

Dual Citizenship 7/7/22 Joan is now a Dual US/Filipino Citizen!
