hi, what do we need to send for I-134?

i have the I-134 form, employment verification, pay stubs, w2s and tax returns for past 3 years.

do we still need to include bank statements?

I didn't include any bank statements with mine but I did get a letter from my bank regarding my accounts, as instructed in the I134 directions here: http://www.uscis.gov/files/form/i-134instr.pdf For mine I included: most current federal tax return and schedules, w2, 3 months of pay stubs, the employment verification letter and statements from my banks regarding my accounts.

Also, for the I134 you only need the most current year's federal tax return and w2 (although I also sent three years worth) We will need the last 3 years worth when completing the I864 for the adjustment of status

Yes, the beneficiary will need all of it except the bank statements during the interview

If you do not need to prove income through direct deposits and are not using cash assets, then you do not need bank statements/bank letter.

If you've met the 125% of poverty guidelines with you current income from your company then no. If you don't meet it and you're using assets then yes along with the bank letter stating when you've opened the account.
