What you have to do is get the DS-3025 transcribed to a I-693 which will cost you money. Cost me $50, maybe if you call them back and explain it that way they may understand. Pritty sure that you need to have the immunization data from the DS-3025 transferred over to the I-693.

In the Bay Area we used Meridian Medical down in San Jose. (1644 Alum Rock Avenue - (408) 729-5800 ). Their charges were reasonable (i.e. did not require another physical, but did require a Flu shot and a few other shots (MMR/TDAP) which appeared on our DS-3025 to require a second round). Total charges amounted to $145 last October.

I called the two civil surgeons in town neither had any idea what I was talking about when I asked about signing off on the foreign medical for K-1 adjustment of status.

They do not know what you are talking about because they do not sign off on the K-1 medical done by the panel physician overseas. If you need to see a civil surgeon, you need to have page 1 and the vaccinations page filed out on the I-693. See this post >

Was the vaccination record as incomplete when she was in Thailand? I know Pui's was because I was with her when she did her medical and they gave a piece of paper showing what they did and that it was incomplete. After she got here we had to find a USCIS civil surgeon by looking at this website https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV. CDN(ON)-USA(VT) listed the ones who do not rip you off. I am not sure how close you are to some of those which is why I listed the site from the USCIS website. What I did was called a few to make sure they were not going to do a full exam which she did not need to have done. The one I went with had us go to a different clinic that charged something like $35 for 2 shots and bought with us the record of the vaccination and then brought that to the civial surgeon's office and they completed the part they needed to complete. We paid the civil surgeon $100 to complete the form to filled. So you might want to ask your normal DR how much the missing vaccinations are or call around to a few places to get a good price and the record of the vaccinations. Good luck, I know it is a few phone calls you have to make but it is worth it. Any questions let me know.

12-06-2012 Pui's Brother unexpectly passes away and we make an info pass appointment and receive an emergancy AP so she can return home. Pui leaves for Thailand for 2 weeks.

12-06-2012 Get a text message and email that she received an RFE

12-12-2012 RFE for original birth certificate. I swear we sent it along with a certificate translation of it.

12-20-2012 Pui returns from Thailand.

12-21-2012 We send the RFE back for with original birth certificate along with a new certificate of translation(I had to wait for her to return for her to sign)

12-26-2012 text and email they have received the RFE.

12-29-2012 Appointment for biometrics is 01-23-2012

01-13-2012 AP is approved.

01-23-2012 Biometrics appointment. Later during the evening the text and email saying the EAD is approved.

01-31-2012 EAD/AP combo card arrives.

05-22-2012 Email and text card is in production!
