DS-260 - new to this form

I am preparing the ds-260 for my neighbor who is petition her 46 year old son and his 2 children. They are in the Dominican Republic. NVC started requiring the DS-260 instead of the DS-230. She was told a few months back the she would send the packet with her I-864 and documents and then the DS-230 with its documents by mail, but now that has changed.

There is a question at the end that reads:

Do you have documentation to establish that you have received vaccinations in accordance with US law?

It is a YES or NO question, answering no, results in needing to explain.

Here is my dilemma; Normally in the Dominican Republic, one would go to a medical exam before the interview where all missing and required vaccinations would be placed. The person would bring whatever medical record they have to show that they have previously received the vaccination, this would result in not receiving certain vaccinations and been bill for the one missing, at that moment.

The son (46 years old) has no documentation in hand to show that he has had any vaccination in the past. Now, one of the children (17) has her record, but they are not sure if they are in accordance to US laws. the other child is a 8 year old.

I am assuming to say YES to all of them, but please can I get some advice.

Another question, before the supporting documents would have had to sent by mail along with the DS-230. This included; 2 Photos, Birth Certificate, Document of good conduct, copy of the passport. The instruction states that if is accepted electronically, the scan the documents and the email them. Is this correct that I no longer of to send them by mail and have to find a scanner?
